Use the NCH MyChart mobile app to access and share digital proof of your COVID-19 vaccination records with a simple swipe of your phone. Whether you received your COVID vaccination from NCH or somewhere elsewhere in Illinois, you can easily access your proof of vaccination as a PDF or QR code in-app or export these to add to your mobile phone’s digital wallet.

New to using NCH MyChart? Register for an account or download the MyChart app on Apple App Store or Google Play. If you already have the MyChart app installed, make sure it is up-to-date to take advantage of this feature.

Mobile & Tablet Access

To access your digital proof of vaccination, you can use the NCH MyChart mobile app.

1. Log into the NCH MyChart app.
2. Select the COVID-19 QR code shortcut icon (top left) from the app’s home screen.
3. Select QR Codes to scan at participating locations.
4. Select Download/Export to download your proof of vaccination PDF, which can be shared by email or text, or export your vaccination proof to your device’s digital health wallet.

Enable Direct Access

You can enable anytime access to your proof of vaccination from the NCH MyChart app’s pre-login screen, without the need to log in.

1. Log into the NCH MyChart app.
2. Select the COVID-19 QR code shortcut icon (top left) from the app’s home screen.
3. Under the Quick Access Options section, enable the “View COVID-19 information on the pre-login screen” option.
4. Log out to view your vaccination proof on the pre-login screen.

Computer Access

You can access your digital proof of vaccination using the NCH MyChart web portal using your computer.Log into NCH MyChart at

2. Select the COVID-19 QR code shortcut icon (top right) from the header menu.
3. Select QR Codes to scan at participating locations.
4. Select Download/Export to download your proof of vaccination PDF, which can be shared by email or text, or export your vaccination proof to a digital health wallet.
