Please have your doctor’s order(s) ready and call 847‑618‑3700 for the following:

• To schedule a test or procedure, including outpatient lab work and imaging
• To pre-register for surgery, endoscopy or pre-admission testing

Monday–Friday: 7:00 am–7:00 pm
Saturday: 9:00 am–1:00 pm
Sunday: Closed
