HMO Patients
Thank you for selecting Northwest Community Health Partners (NWHP) as your managed care insurance provider for Site 503 (HMO Illinois, Blue Advantage and Blue Precision) and Site 770 (Medicare Advantage). NWHP has a large network of more than 400 primary care physicians and specialists, all affiliated with Northwest Community Healthcare in Arlington Heights.
BCBS HMO requires you to select a primary care physician (PCP). Your PCP plays a vital role in managing your care and will be responsible for coordinating all your healthcare needs, including referrals and necessary authorizations within the NWHP network.
