Community Health Worker (CHW) 

Our bilingual, Spanish-speaking Community Health Worker (CHW) offers free, ongoing support to Spanish-speaking patients at our hospital and in the community. She meets patients identified by our physicians and staff who need extra support bedside and then provides additional post-discharge support. She provides assistance with understanding, complying and overcoming barriers with discharge plans, basic education and referrals to community clinics and other social services. In the community, she provides support and individual or group education to discharged patients and community members.

Our CHW also provides labor and delivery hospital tours in Spanish and English for expectant mothers receiving prenatal care at our community clinics.
