Pre-Admission Testing
At Northwest Community Hospital, we want you to be as informed as possible before your surgical procedure. So, before you arrive, please read through the following:
Registering for a Surgery
A registration representative will call you prior to surgery to set up a pre-admission testing appointment, if necessary, and to pre-register you for your procedure. You can contact the pre-registration department at 847‑618‑3775.
Pre-Admission Testing Location
Enter the building using Entrance #4. Walk down the hallway past the hospital reception desk on your left. At the end of the hallway, before you reach the South Elevators, signage will direct you to the PAT location. Please stop at the front desk to register, and a PAT team member will be with you after that. View the campus map. Please bring the following, if applicable:
- Your insurance cards
- Written referral form
- Pre-certification number from your health plan
- Copayment
- All of the current medications you are taking in their original bottles—this includes prescriptions, over-the-counter medicines, herbal supplements and vitamins
- A list of your allergies
What to Expect During Pre-Admission Testing
During your pre-admission testing appointment, you will meet with a registered nurse (R.N.) to complete your health history and receive detailed instructions about your upcoming surgery. You may also meet with the nurse practitioner or anesthesiologist, and have all necessary testing completed depending on your medical history. You may need to have the following taken:
- Blood test
- Urinalysis
- EKG
- Blood pressure
- Chest X-ray
Parking
You can park in Parking Garage B or the adjacent lots. Free valet parking is also available at the North Entrance.
