The NCH Medical Group recently received the highest level of recognition by the National Committee for Quality Assurance (NCQA) Patient-Centered Medical Home (PCMH) for its emphasis on care coordination and communication with patients.
NCQA level 3 PCMH recognition is the most widely adopted model for transforming primary care practices into medical homes. Research shows medical homes can lead to higher quality and lower costs, and can improve patients’ and providers’ experience of care.
“This recognition from the NCQA confirms that the NCH Medical Group is committed to providing the highest caliber of care available,” says Susan Nelson, M.D., Vice President, Physician Operations, at NCH Medical Group. “With the PCMH model of population health management, the NCH Medical Group is taking a proactive, rather than reactive, approach to wellness, prevention and chronic disease management, and improving the health outcomes of our patients.”
NCQA PCMH recognition involves submitting documentation that illustrates compliance for six standards: patient-centered access, team-based care, population health management, care management and support, care coordination and care transitions, and performance measurement and quality improvement.
Receiving this recognition solidifies the NCH Medical Group’s commitment to providing high-quality care and looking for opportunities to improve outcomes, access, and patient outreach.
Additionally, receiving this recognition helps position the NCH Medical Group with upcoming changes in quality reporting within the Centers for Medicare & Medicaid Services Quality Payment Program.
